It works the same as other picture datatypes that support multiple pictures. You use PDTA_GetNumPictures to get the number of pictures and then then if this is &gt; 1 you use PDTA_WhichPicture to specify which picture to get (0 being the first (default), 1 being the second and so on). 
 
See the picture_dtc.doc autodoc for more details on how to use these tags.
